About Transitional Care
At Hope Care, our Transitional Care services are designed to support the person as they transition from one care setting (like a hospital) to another care setting (like their home). Hope Care services vary by the persons need, but are designed to enhance the person’s health care experience, improve the person’s health, and improve the person’s quality of life. Transitional Care services are time limited, but Hope Care provides other services that may be utilized to maintain the person’s goals.
