At Hope Care, our Palliative Care services are tailored to meet the need for people with a serious illness, disease, or condition. The Hope Care team works with the person and their family to improve the quality of life for both the person and their family. Palliative Care has no time limit because the services act to fill the gap for patients who need care at various stages of their serious illness, disease, or condition. If you decide to pursue treatments for your illness, the Hope Care team will be there to provide support.

What services does Hope Care provide?

Hope Care utilizes providers with a range of services designed to address the physical, emotional, and spiritual issues that arise from a serious illness, disease, or condition. The provider works with each patient and their family to generate a comprehensive care plan based on treatment goals and preferences. Hope Care provides 24/7 access to providers, nurses, nursing aides, spiritual coordinators, social workers, and volunteers.

Where are services provided?

Hope Care provides services at any location the person considers home.

Who pays for services?

Typically, the persons primary insurance will cover the cost of services. Hope Care also has private pay and sliding scale payment options.
